PEAP Form Instructions for Program and Fellowship Administrators
Since July 2025, the administration of the detailed PEAP form has moved to a digital format using Elentra. The PEAP form has been built to match the paper form and should not be modified by any program. If you need a paper copy to review the form content, a PDF is available at the bottom of this page.
When you need to send a detailed PEAP form to an assessor, please complete the following steps.
- Go to Admin > Assessment & Evaluation.
- Select Trigger Bulk Assessment (even if you have only one task to send, you can still use the Bulk Trigger Assessments option).

On the Generic Assessment Tools screen:
- Select Program
- Browse Assessor(s): Add the name of any assessor you need to complete the PEAP
- Browse Learner(s): Select a curriculum period and then search for the relevant trainee and check the radio button by their name(s)

Enter the relevant start and end dates for the task (e.g. beginning and end of 4-week period). Note, these dates can be unique per trainee.
- Use the Add Assessor option if you’d like to an add another assessor and trainee(s).
- Check off “These assessments are mandatory.”

Under Prompted Response Notifications, the form itself has been set up to send notifications for prompted responses to Program Coordinators, Program Directors and three Custom Reviewers: Leslie Barroso, Christine McCallum and Laura McEwen. These settings will be used by default.
- If your program would like to alter the recipients of prompted response notifications (e.g., to remove PAs), check off "Prompted Response Notifications" and make your selections, but please note, you MUST still include Leslie Barroso, Christine McCallum and Laura McEwen as Custom Reviewers. The prompted response notifications you set when triggering the tasks will override the form settings.
- Click Get Tools.

Search for PEAP and look for the appropriate week Pre-Entry Assessment Program (PEAP).

- Click Send Assessment.

Optionally enter an Assessment Cue. This will display at the top of the task for the assessor and will also be visible to the trainee once the task is complete.
- Click Send Assessment.

The task will send to the assessor immediately.
Tracking Task Completion
If you wish to check on the status of the PEAP tasks you have sent, follow these steps:
- Go to Admin > Assessment & Evaluation.
- You will land on the Outstanding Tab for Assessments.
- Search for PEAP.

Any outstanding (e.g. to be completed) forms should display. The ‘owner’ is the person who needs to do the task. You can check off the reminder column for any that you want to remind people about.
If you want to quickly see all completed PEAP tasks, switch to the Completed tab and perform the same search.

Additional Notes
- This form is not available on demand for trainees or faculty to trigger. It must be sent out by a PA. This is to ensure the correct people are using the form at the correct time(s).
- All items on the PEAP form include prompted response notifications for any Unsatisfactory, Below Expectations or N/A or Unable to Assess response.
- The Final PEAP form will be emailed to your program by the PGME Office. If you don’t receive a copy, please contact PGME.
